Burnout in the Nursing Profession is Not Yet A Priority
Nursing burnout is still seen as a “soft” topic. How do we know that? Well, it is still a dilemma that organizations do not know what to do with. That and it is often bumped from the agenda.
Think about it- what comes first? Hospital acquired infections? Fall rates? Suicide screening? Catheter acquired or central line infection rates?
You know the drill. It is those things that we are measured on. The things that cost us money or hurt the organizational budget. Benchmarks that are tracked and then compared across organizations, counties, and even states.
Nurse burnout gets bumped from meeting agendas. Resources such as time or money are not allocated to burnout. The nursing staff keeps working harder and the job continues to get tougher. What is a nurse to do?
Nursing Burnout Meets Big DataOrganizational leaders and c-suite executive types speak in data. They prefer one page handouts, bulleted lists, and facts/figures. A lengthy, wonderfully written narrative just is not going to get through to them. Quite frankly, they don’t have the time to read it!
So, as an individual nurse, what can we do to help nursing leadership realize that burnout must be a priority? Show them the data!!
I use a classic example in my Nursing from Within keynote presentation to highlight the importance of burnout and how it affects the bottom line.
Linda H. Aiken, Ph.D., FAAN, FRCN, RN is a nurse researcher who is currently the Director for the Center for Health Outcomes and Policy Research and a Senior Fellow of the Leonard Davis Institute for Health Economics. In her and her research team’s work on nurse burnout, staffing, and patient infections, they showed a relationship between nursing burnout and patient infection cost savings.
From the article published in the American Journal of Infection Control:
“In a multivariate model controlling for patient severity and nurse and hospital characteristics, only nurse burnout remained significantly associated with urinary tract infection (0.82; P = .03) and surgical site infection (1.56; P < .01) infection. Hospitals in which burnout was reduced by 30% had a total of 6,239 fewer infections, for an annual cost saving of up to $68 million.”
Let’s read that number again… an annual cost saving of up to $68 million!!!
Maybe sharing this article with the decision makers will help them open up to the fact that nursing burnout is NOT a soft topic.
Patient Outcomes Via Nurse EngagementA burned out nurse is not happy at work. He or she is disengaged. The nurse who is experiencing burnout in the nursing profession is suffering both personally and professionally. And a nurse like that— well, you can guarantee that they are affecting the patient experience.
If we are looking for positive patient outcomes, safety and quality indicators, and patients being satisfied with the healthcare experience… we need a nursing staff who is ready, willing, and able to take care of patients. A nursing group who feels excited to get into work and has the energy to care as much as it takes.
And yes, we need organizations to support their nurses. However, we also need nurses who are accountable to their craft.
Nurses who choose healthy behaviors. Who say “no” to people from time-to-time, setting healthy boundaries. We need nurses to choose themselves and their own-welling, without feeling guilty or selfish. We need each and every individual nurse to focus on solutions, appreciate the positive, and learn from every transformative experience.
If we want positive patient outcomes, then we need healthy, happy nurses to care for them.

Pushing Harder Does Not Help Nursing Burnout
A challenge many of us as nurses face is the workaholic syndrome. In fact, I talk about this in my book and presentation, “Stop Nurse Burnout”.
Being a workaholic as a nurse is a bit unique than any other profession. I will use my husband, Drew, as an example here.
Drew is a heating and air-conditioning technician. He drives a big van, uses a lot of tools all day, and works with his hands. When Drew comes to the dinner table with me at the end of the work day, does he bring his hammer, level, or drill? Of course not! But as a nurse, this may or may not be the case.
We teach patients using our voice. We turn, lift, clean, and fix with our hands. We empathize, advocate for, and care with our hearts. And- when you are a nurse, are you ever really “off”?
Your family hears that you have graduated from nursing school. After the initial “congratulations” and celebrations wear off… then what comes next? The constant phone calls, texts, or emails saying, “Hey, you’re a nurse right? What do you think this is…”
People can always count on a nurse! It is as if our workday never ends. We care for patients, our own families, and our greater communities at large.
But if you are reading this and feeling the stresses of nursing burnout… and you think that putting the nose down and working harder will just help you push through… you are sadly mistaken, my friend.
Working harder, pushing through, or simply ignoring how you are feeling and hoping that it will go away will not work out. Burnout will eventually consume all of your life. You may become physically ill or emotionally stressed. Work will no longer be fun and just getting yourself there will require every bit of energy in your body.
There is no working through nurse burnout.
Choosing Burnout Over and OverThe other struggle we face as nurses is the insanity definition stated above. In fact, here’s a real-life example of this.
As a keynote speaker, I get the honor of giving talks to nurses and nursing students across the country. This past year was no different than any other- only that I presented more talks than ever before!
After a talk in Oklahoma, I was contacted by a nursing student in the audience. We got on the phone for a quick chat so that I could answer her questions and give her more guidance than was able to during a 60 minute audience-style presentation.
One of her questions was spot on. She asked me, “Do you ever get people who ask you about how to eliminate their stress? And what do you do when they don’t follow up? I mean, how does that make you feel? How do you go on with what you are doing??”
This nursing student is wise beyond her years! This happens to me- a LOT.
After getting off of the stage at a talk in Indiana, a nurse came up to purchase Nursing from Within. She said, “What do you do when you do not want to do nursing anymore?” Since that question could take TONS of time to sort through and answer- I encouraged the nurse to email me and we could set up a time to talk by phone.
She never did.
As stated above to the nursing student asking me questions- this kind of thing happens often.
Nurses tell me how unhappy they are, however for some reason, are unable to make lasting change. One thing I have noticed- even in my own life- is that as human beings we prefer the comfort of the known. And the comfort of being comfortable.
So, even if a nurse is experiencing nursing burnout, sometimes making a change is harder, more difficult, scarier, overwhelming, and any of the above than not making the change. The not making the change is known. We are used to it. It is familiar to us.
If you are experiencing nursing burnout, and you REALLY, really want to see different results— then it is TIME to take action. Different action. New action. Action that you are even a bit afraid of.

3 Reasons Why Nurses Should Embrace Burnout
To my own surprise, I actually had already written to this topic last year. In an article titled, “3 Reasons Why Nurses Should Embrace Burnout“, I provided the nurses reading with some of the reasons that burnout can actually support us nurses and the nursing profession overall. You can read the entire post, but just for a quick recap here… the 3 reasons are:
- Burnout helps you grow. Burnout allows you to help others. Burnout propels you to your passion.
Pretty cool stuff, right? OK, so this post will not cover the above said reasons again. However, what I do want to share is some parts of my “Stop Nurse Burnout” book in order to help you understand that burnout really does have a highest and best use. Here’s more on how nursing burnout is good for you.
Burnout Marks the Normal Transitions in LifeIt seems that people don’t often get their act together without burnout. Burnout marks the place where you have followed someone else’s path long enough that your body is telling you things must change in ways you can no longer ignore. You finally pull your head up and realize beyond any doubt that this is not your path.
If you continue on this route, you will violate your own values, deny the people you love, and most likely slide into a “life of quiet desperation.” Burnout gets you to finally say, “I can’t do this anymore. There has to be another way.”
Unless you turn the downward spiral around before this crisis point, nurse burnout will eventually push you to near breaking. I hope and pray that when you reach that point, you bend and spring back rather than snap like some of the unfortunate among our brothers and sisters. Alcohol, drugs, depression, other mental illnesses, and suicide are all complications I hope you avoid. The good news is that only a tiny minority of nurses will face those trials.
The rest of us are left with a scene Robert Frost described best. You can see that there is more than one option available. It is clear you are free to choose between the two. Either choice has its own set of unique consequences.

At this point, when you have no energy left to keep putting out the fires of other people’s demands and priorities, something important happens. You realize with undeniable clarity that you have other choices available, and the path you have been on all this time is simply one choice among many.
Stop Treating Burnout Like it is BadWe need to stop treating burnout like it is something that is bad for us. Or that we did something wrong to get to the state of nursing burnout.
By continuing to treat burnout as though a person did something wrong or was infected with some kind of terrible virus- we only stigmatize those nurses further. Each and every one of us will likely experience burnout as a nurse. It is simply the nature of the job. Hey, I have. More than once!
Instead of viewing burnout as though we are infected with some kind of horrible disease, put it to its highest and best use.
Burnout is here to nudge you onto a different path … one with more purpose. You really can decide what you want and go get it. You can set your own tracks from now on. You can use your discomfort to fuel your change. And hey, if you are open to change then you might actually see that burnout is good for you!

The State of the Science
While there is a lot of evidence related to physician burnout (e.g. the prevalence, rates, and consequences of), this is not the case in the nursing literature. Not that there are no articles related to nursing burnout. Trust me, there are many. But the issue relates more to the fact that the science does not tell us one story.
In fact, the numbers are all over the place!
Don’t believe me? Head over to PubMed or your favorite scholarly search engine and type in the term “nurse burnout prevalence” and see what you find. In one of my recent keynote talks, here are the studies I have on my prevalence slide:
Oncology Nurses 21.5 % (Clinical Journal of Oncology Nursing, 2010) Nurses in England 42% (BMC Nursing, 2011) ICU Nurses in Portugal 31% (BMC Anesthesiology, 2013) NICU Nurses 25.9% (BMJ Quality & Safety, 2014) Nurses in Mainland China 34.9% (Nursing & Health Sciences, 2015) Emergency Nurses 26% (International Journal of Nursing Studies, 2015) Nursing Homes in France 40% (Journal of Clinical Nursing, 2017)And then there is this recent systematic review on the prevalence of burnout which outlines that burnout does exist in various roles of healthcare professionals. But again, this is just one subset (palliative care). So yes. Nurse burnout statistics are all over the place, however maybe one thing that the literature can agree upon is that it does exist.
Evidence Requires Measurable Outcome DataThe nursing studies that are published related to nurse burnout have limitations such as low sample sizes and weak study designs. If we do not have the literature to support nurse burnout, then we will not get the support from decision makers in power.
Poor studies- do data. Without the outcome measures, there are little interventions that can be put into place. Healthcare decision makers need to see the data. They are all about the numbers. Whether we like it or not, healthcare has become likened to a business. Without money, new interventions that can help alleviate nurse burnout, will not be adopted.
I do think that we can agree that nurse burnout does exist. Again, if you go to your favorite search engine and type in the term, you will receive lots of articles related to the topic of burnout in nursing.
What I also think is that we do not need any more of these smaller, specialty specific studies. The ones where the team uses the Maslach Burnout Inventory to measure burnout in nurses pre- and post- some intervention. We get it. Nurses are burned out. In all jobs, all locations, and all over the world!
Meaningful Research Across Practice SettingsWhile I cannot start to give advice or suggestions related to specific study design (that is not the point of this blog or the Nursing from Within™ work), I do want to put a charge out there to anyone reading who is involved in world of scholarly work to look at nursing burnout.
We need robust evidence that nurse burnout is linked to patient safety and outcomes. Yes, process measures such as these are harder to measure and difficult to link in a one-to-one causation. For example, it is difficult to say that nurse burnout and burnout alone is what causes patients to fall (as an example), however we need study designs that do their best to look at burnout and the impact it has on patient safety and quality.
Nurse and researcher, Linda Aiken, PhD, FAAN, FRCN, RN, has published extensively on nurse and patient outcomes related to job dissatisfaction and burnout. In fact, in a 2012 article in the American Journal of Infection Control, study findings reported that “In a multivariate model controlling for patient severity and nurse and hospital characteristics, only nurse burnout remained significantly associated with urinary tract infection (0.82; P = .03) and surgical site infection (1.56; P < .01) infection.”
So, it can be done. And needs to be done more often, linking nurse burnout statistics to outcomes that healthcare administrators are interested in. Then, taken a step further.
Burnout is real. It exists and impacts nurses and patients we care about. Once we realize this, then we can start to measure the impact that we can have on nursing burnout. But again, we need to focus on implementation projects that study the barriers and facilitators to why interventions impact and relieve nurse burnout across the spectrum.
Work is to be done, but there is hope. Nurses across the world are doing great things to move the nurse burnout needle. Will you be one of them?

WHAT IS THE DIFFERENCE BETWEEN BURNOUT AND NORMAL STRESS?
Stress is common. It’s a normal aspect of modern life. It’s nearly constant for a nurse on the job. With stress an ever-present condition for every practicing nurse, how can you tell the difference between the “normal” stress of practicing nursing and burnout?
Before we get to that simple answer, let’s remember that stress is not always a bad thing. Without an appropriate amount of stress and challenge, the average human lapses into boredom and inactivity. The stress and responsibility of caring for patients is part of the motivation to do your best in every case. The perfectionism it inspires is a good thing for the patient. However, too much of that good thing is bad for the nurse.
So, what is the difference between the normal stresses of the healthcare workplace and the syndrome of burnout? It’s really important that you and I get on the same page here in regards to this critical distinction.
Burnout is not about what is happening to you.
The difference between burnout and normal stress is not the nature of the external events that you find stressful. What one person finds to be stressful, a different person would experience as stimulating, exciting, and even fun. Everyone’s situation is unique.
Burnout is about whether or not you can cope with the things you find stressful.The difference between simple stress and burnout is your ability to respond to and recover from the energy drain caused by the things that stress you out. Here’s a simple definition of nurse burnout.
With normal stress . . .. . . You are able to recover with time off. You are able to maintain “energetic homeostasis.” You can recover your energy, enthusiasm, and drive with adequate rest. Your energy may be higher some days and lower on others. You notice the day-to-day fluctuation in your energy levels; however, you don’t feel overwhelmed or incapable of recovery from week-to-week.
With burnout . . .. . . You are unable to recover. Your energy enters the pattern of a relentless downward spiral. There comes a time when you notice you are not bouncing back, and you dread heading back into work. In most cases, a person suffering from burnout will eventually say something like, “I’m not sure how much longer I can go on like this.”
The only reason burnout rates can hover in the 30 percent range for decades and not destroy the profession of nursing is because we’ve been work-hardened to tolerate this energetic drain. Let’s face it: our training is a gladiator-style survival contest. We are conditioned to be able to cope with burnout better than all but a handful of other service-oriented professions—law enforcement and active duty military come immediately to mind. We will talk much more about this below when we discuss the pathophysiology of burnout.
This high-level overview will make one thing obvious. If you are burned out or headed in that direction, there is something about the stress of your job and the things you do now to recover that are not working. You are leaking energy and not replenishing it, like a boat with a hole so large that bailing as fast as you can does not keep you from sinking.
This is why a vacation, sabbatical or retreat will never cure burnout. Simple time away from work will only provide temporary relief. If you don’t fundamentally change how you are working by taking different actions, when you return to the workplace after your break, the excess drain begins anew. There is still a hole in your boat.

A Real Life Burnout Story
I have interviewed dozens of nurses on the Your Next Shift nursing career podcast. Several guests have answered the “failure / lesson learned” question in a very similar way. When asked, they tell me…
Elizabeth, I left a job because I was unhappy and experiencing burnout. And sadly, in a very short time (one interview guest said it was as short as 3 days!) I was just as miserable in my new job as I was in my old one.
The guests of the Your Next Shift podcast each speak to a common theme.
You cannot fix #nurse #burnout simply by changing #nursing jobs. Here is a specific example.A nurse working in the critical care area started to feel nurse burnout. She decided to make a change and move to procedural nursing. In just a few weeks time, she was just as unhappy (if not more so) in her new job. The nurse went back to her old unit in critical care, asking for her previous job back. Happily, she was able to get her old job. But this experience taught her that just changing practice areas was not fixing her burnout issues.
We Cannot Simply Put a Band-Aid on BurnoutI think back to when I was working inpatient psych. I would go away on vacation and hope that I would feel better, upon returning to work.
Guess what?
I didn’t.
It would take me two days of the vacation just to de-stress from the job. Then I would enjoy about 2-3 days of the break. However, as my vacation ended and it was time to get back to work, I spent the final two days of my break dreading the experiencing of returning to my job.
#Nurse #burnout cannot be solved with a break or a change in scenery.Why is that?
Burnout is a multifaceted dilemma. It requires a variety of strategies that become routine habits. And the cause is not unidirectional. Meaning, you cannot say that just ONE thing caused your burnout to occur.
Burnout affects us on many levels: Physically: we become tired, run down. The job gets hard and our body suffers. We have little time to spend on self-care and so we feel cranky, tired, or depressed. Mentally: we experience a disconnect. The reason we go to work- the patients and their families- become more of a number than a person. We start to feel cynical, maybe even speak sarcastically about work. Spiritual: there is no more joy in the job. Our meaning is lost. We are not sure why we are even going into work. It is hard to feel excited by the thought of heading into another shift.All of these can be felt in some combination that is uniquely your burnout experience. And trust me, you are not alone.
When I realized that I was experiencing burnout, my main issue was feeling and asking… “What’s the use?”
I would say or think things like:
What’s the point? Nothing I do is even making a difference. Who cares? No one listens to me or my ideas anyway. I am so frustrated. I can’t stand another day! Why even go to work? I just don’t fit in there anymore. Burnout Can Help UsSo, it sounds quite bleak, right? It does not have to be.
I have gone through this. On more than one occasion, honestly, and come out the other side.
The very first step is awareness.
Recognize what you are feeling and be gentle. Know that it is OK, even normal. There is nothing wrong with you and now that you are aware that you are experiencing nurse burnout- choose to take action.
Burnout does have a highest and good use, as I talk about in my book “Stop Nurse Burnout“. It can propel you into action. By experiencing burnout and deciding you want something different, you can find a new nursing career that fulfills you. Going through a challenge such as nurse burnout can motivate you to make changes that allow you to love your nursing career again.
Don’t Mask the BurnoutFinally, don’t avoid or cover up your burnout. If burnout is what you are experiencing, do not attempt to hide it or pretend it is not there.
Masking burnout with alcohol, drugs, sleep, or shopping will not help you. It will only make things worse.
You need to realize that burnout is what you are experiencing and then take action to seek help. It will not just go away on its own.
Since burnout originates from a variety of sources, you are going to have to put multiple strategies into place to cope with and ultimately rid yourself of this experience. And then… maintain the strategies so that burnout does not sneak back up on you.

Nursing Solutions for Burnout
Got nurse burnout? And if you did or do, who do you expect to fix it? Your employer? Your boss? What solutions for burnout have you come up with up to this point?
Sometimes when we have something happening in our life that we do not enjoy, we look all over the place for help. I can speak to this because I have done it myself.
My Burnout StoryI go into my nurse burnout story in my book, Stop Nurse Burnout, so I will not go into the FULL details here. However, just as an overview and how it pertains to this blog post...
Yup, I am raising my hand here. Back when I was experiencing the most miserable point in my career- my all time low- I blamed everyone and anybody. I looked left, right, up, and down for other people to point fingers at. I wanted people to give me solutions for burnout.
It was my hospital that did not care about me. My nurse manager who was unfair. The people I worked with did everything wrong. I was working on the most terrible nursing unit ever. And guess what?
Turns out that this was NOT the case! In fact, as of this blog post's writing, I have been back to that unit. I am colleagues with my past co-workers. I sit on committees and have worked on projects with the very people I expected to fix my nurse burnout problems.
We Expect Others to Find Solutions for BurnoutA few months ago, I wrote a post called "The Way We Think About Nurse Burnout is Broken." In that article, I articulated that there were many issues with nurse burnout and shared with my readers that I would tackle each of the bullet points one month at a time, creating a burnout series.
This month we are covering the "We Expect Others to Fix It" topic. The way I described this challenge was in the following:
"While I do believe that the nursing profession, hospital systems, individual organizations, and nursing leadership and management need to support the individual nurse in their well-being- it is ultimately up to the nurse to be accountable for their own health. A nurse who thinks, feels, acts as though they are the victim of burnout and that someone else caused them to experience it- is much less likely to turn things around and find any relief from the stressors of burnout."
Choose Personal ResponsibilityNow, I do realize that the above may upset some nurses. I get it. Conditions are harsh. Stigma is real. It may even be unsafe where you work. I can empathize with that.
And, at the same time, I do believe we have a choice. We can choose to continue to fall victim to the nurse burnout conditions that we experience in our workplace. Or we can choose to do something different. No one can live your life for you.
Stated similarly from the above, a nurse who feels that they are the victim of burnout because of some external circumstance (e.g. organization, department, unit, management, colleagues, or patients) and does nothing about it... well, then I do believe that is on them.
We all wake up in the morning and experience choice. Millions of choices a day. We can choose to continue employment in a terrible job or we can make a change. For me, because I value my life and want to enjoy as much of it as humanly possible, I would likely choose the latter.
Fix Your Nurse Burnout SituationOK, let's imagine that you are still reading along with me and have not thrown your computer across the room. Let's move forward then with some strategies. I would like to offer you support in finding solutions for burnout, should you want or need them.
Here Are 4 Steps to Take to Fix Nurse Burnout
1) Assess Your Situation.How bad is it? Are you at your wits end? Is there no one safe to talk to at work? Do you feel that if you admit to feeling burned out you will be ridiculed, excluded, or even persecuted?
If that is the case... then it is time that you look for a new nursing job! I mean, right now. Get out of there. And fast. This is not a supportive work environment and it is unhealthy for you to be there another minute.
2) Assess Your Self.This goes back to what I shared above about my own burnout story. Remember, I was the one pointing fingers at everybody else. With time and a fresh perspective, I realized... my goodness. That place wasn't so bad after all! It was me!!
Now, I am not saying it is ALL you. Sure, there may be some issues with your workplace. No job is 100% perfect. Let's be real. Every workplace has stress.
My point here is take time to do some introspection and self-reflection. Do you always see things as negative? Are you constantly viewing the bad in other people? Is there something that you could do to change your thoughts, feelings, and behaviors?
If this is the case, you are not alone. In fact, I have dedicated my entire career here at Nursing from Within to helping nurses shift their perspective. I truly believe that when we change what is on the inside, we change the outside. By shifting our thoughts and feelings, we can show up at work a different person. I have done this myself and it feels AMAZING each and every day! (Let me know if you need help.)
3) Commit to A Self-Care Practice.A routine self-care practice. And notice above I said "a". I mean it. Just one.
When we get the idea in our head that we want to make a change in our lives, we try to do too much too fast. And then, guess what? We burn ourselves out on that! Then we slide back into old behaviors, making no lasting changes at all.
So, if you want to fix your burnout situation, do one thing at a time. I would recommend grabbing a copy of Stop Nurse Burnout and finding a burnout prevention strategy that works for you. There are dozens of them in the book and no one strategy is for everyone. Rather than me listing them ALL out here, get a copy and find what works for you.
4) Get A Buddy.There is a reason that 12-step programs have had so much success over the years. One of the main components of programs like AA or NA is that you have a sponsor. Another one is that you go to meetings and share.
If you want to find solutions for burnout, you need to tell someone that you are doing it. Now, I would be careful who you tell. Some people do not have your best interests at heart. And they will try to sabotage you, whether it be through ridicule or disbelief. That being said, I am sure that you can find ONE trusted person in your life.
An added bonus would be if you were able to find another nurse to partner up with. That way the two of you can choose a burnout prevention strategy (it doesn't have to be the same one for both of you) and then check in from time-to-time to give progress reports on how you are doing.
Accountability is crucial. I check in with other nurse business owners, my business coach, and even my husband on a regular basis. This keeps me moving forward with my business goals. It keeps me taking action and making progress towards the things that I want out of life!
